In the original version of this article, the statements of Ethical Approval and Informed Consent were missing. The complete Declarations section is given here: The study has been approved by the Office for the Protection of Research Subjects - University of Illinois at Chicago (research protocol number 2006 − 0201). Informed consent was obtained from all adult individual participants included in the study. Child assent was obtained from minor participants as well as active consent from their parents or legal guardians. These procedures were in accordance with the institutional regulations at the time of data collection and approved by the local IRB.
