Abstract
Aim of the study was to evaluate the long-term outcome of donor iliac angioplasty and crossover femorofemoral by-pass. Material and methods. We reviewed all patients over a 10-year period undergoing femorofemoral bypass surgery. The study group comprised 99 patients who were followed for a period of 16 years. Patients were divided into 2 groups: standard femorofemoral by-pass, and donor angioplasty with femorofemoral by-pass. Comorbidities were evaluated considering the impact on graft patency. Results. Success by lifetable analysis was non-significantly different between groups, up to 99 months with 82% patency for donor angioplasty and cross-femoral by-pass and 54% for patients undergoing standard b-ypass surgery (SE < 10%). No significant patency difference was found considering the presence or absence of comorbidities including: coronary artery disease, hypertension, smoking, gender, need for distal by-pass or indication for surgery. Diabetics had a higher patency after 90 months than non-diabetics (90% vs 63%, p<0.05). Conclusions. Donor iliac angioplasty with cross-over femorofemoral by-pass is a good option for patients with severe occlusive disease of one iliac artery, and contra-lateral disease amenable to angioplasty who are not ideal candidates for aortic procedures. Long-term results are not effected by angioplasty of the donor limb.
